A Thyroid Cascade or Separate Tests? Clarify the Rules First

A thyroid “cascade” or reflex order usually describes a testing sequence: an initial result can trigger additional testing under predefined rules. The exact rules belong to the laboratory and order. The name alone does not tell you every possible component or charge.

Ask for the actual sequence

Find out which test is performed first, what may be added, and whether the laboratory publishes the algorithm for that specific order. TSH, thyroid hormone measurements, and antibody tests have different roles; a cascade is a workflow connecting selected tests, not a claim that all thyroid problems can be resolved automatically.

If the clinician has a specific reason to request a particular assay, ask whether the cascade includes it under the circumstances that matter. Do not replace an ordered test with a differently named package just because both contain the word thyroid.

Compare the practical consequences

Bring context before the algorithm runs
